As a primary care provider (family doctor or nurse practitioner) you can fax a referral letter to us at 1-833-997-1392. Include the patient's contact details especially their email and phone number so that we can contact them if their referral is accepted·
When a referral is accepted by the specialist, a link will be sent to the patient to register on the Jane platform. A patient can select an appointment on the specialist’s calendar after registration on the platform.
The specialist will see the patient through a video or telehealth consultation. At the end of the consultation, a report will be sent to the referring provider. If a prescription is required after the consultation, the specialist may send a prescription to the patient’s pharmacy if it's urgent. Otherwise, we would prefer that the patient's primary care provider handles all prescriptions. The referring provider and specialist can continue communicating back and forth if necessary to execute or fine-tune the patient’s treatment plan. A repeat consultation should not be expected until after six months. This is to enable us to get through our long waitlist. An earlier re-referral may be entertained in exceptional circumstances. Ideally, the patient will be seen for a maximum of two consecutive appointments to conduct a full assessment, following which a report will be generated and sent to the referring provider.
